Immunizations

     If you have been notified by the school nursed of a  required immunization please provide the proof  by that due date. The Department of Health requires most immunizations records due by October 30

Hearing and Vision Screening

Hearing & Vision Screening are required on all Pre-K Kindergarten students and all students in grade 1, 3, 5, 7 and 9 by the end of first semester.
All new students are required  to have hearing and vision screening.
Parents or teachers may request a hearing or vision screening

Health Visit Referral Form

If your child has been to the nurse or the office for a injury or illness please look for the Health Visit Referral Form. It is carbon copy yellow half sheet of paper.

Vision Failure Notice

If you receive a vision failure notice and cannot financially see an eye doctor please contact the school nurse for help.
