MWPISD Athletic Paperwork
(Required To Participate In JH & HS Athletics)

Athletes, we are transitioning to paperless files for the 23-24 school year. This means that instead of signing 7 individual forms, they have been condensed into 3 and will be completed online using RankOne. Please see Doc Chavez for an instruction sheet to have forms completed by your parents or if you have any questions. All forms *MUST* be completed before you will be allowed to participate in any sport next year. 

Mass physicals will be held on Tuesday, May 9, 2023. The cost will be $25 cash or check only. Time and location TBA.

• An Athletic Physical is required for participation in any UIL sport at MWPISD.
• It is the athletes' responsibility to obtain an athletic physical before they can participate.
